The Philippine Consulate General in Jeddah stresses to all Filipino citizens whose passports were issued under an ASSUMED NAME that in order to obtain a new passport that shall reflect an applicant’s true, correct and lawful identity, all applicants will be required to submit the documents prescribed for first-time passport applicants, as follows:

  • Personal Appearance
  • Duly-accomplished Passport Application Form
  • Three (3) photos 1.77\"x1.37\"(4.5 cm.x3.5cm.) with royal blue background. Picture must be recent, taken within a period of 6 months.
  • Original Birth Certificate duly-authenticated by the Department of Foreign Affairs in Manila
  • Must present the original old Passport with photocopy of pages 1 and 33.
  • Fee: SR240.00

These will conclusively establish their identity pursuant to Article 5, Section 4 of Department Order No. 11-97, but will be without prejudice to the filing of a case against them for violation of R.A. 8239.
