1. Present the original passport of the visitor.

2. Fill-up the special request form for extension of visa with the picture of the visitor, and the form must be stumped by the Treating Hospital.

3. Present the Medical Report from the hospital and the date of visitation.

4. Present check with an amount of SR100.00 under the name of the visitor for Saudi Monetary Exchange.  After the completion of all the requirement papers and the submission of the (said) request, the requester must wait the responsible employee to receive the official documents after the extension of the visa.
